Telephone Calls

The more information you are able to provide to our receptionist when you call, the better we will be able to meet your needs. Whether your call refers to current symptoms, an appointment or a billing matter, it will be directed to the appropriate staff member to handle your needs. Any information we collect is strictly confidential. We follow HIPPA guidelines and are unable to provide patient specific information over the phone.

After Hours Care

Our answering service is available 24-hours a day to assist you in case of an emergency. Should a dermatologist be unavailable, please contact your primary care physician or go to the nearest emergency room.

Prescription Refills

Prior to your appointment, check your medication supply so you can arrange for refills while visiting the office. Please allow 24 to 48 hours for phone refills.

Your First Visit

Before your appointment, please fill out the appropriate "intake form" found in the upper right hand corner of this page. If this is your first time here or if it has been longer than 3 years since your last visit, please fill out the "New Patient Intake Form".

Existing Patients

If you are an existing patient and have been seen within the last three years, please fill out the "Existing Patient Intake Form". Once established, you will be required to fill out the "Existing Patient Intake Form" each time you are seen. This information is HIPPA compliant and secure. Please bring information about any medications that you are currently taking (drug names and dosage information). Also bring your insurance card(s) and legal photo id with you to your appointment. information regarding your insurance coverage. If you are unable to fill out the intake forms ahead of your appointment or your insurance or any information has changed, please arrive 15 minutes early so we can update our files.

Our medical records are strictly confidential. We will not release any information unless we have received written consent from you. We follow HIPPA guidelines and are unable to provide patient specific information over the phone.

Insurance and Payment Information

All Co-pays and deductibles are due at the time of your visit. While every effort is made to obtain accurate eligibility and benefits, this is not a guarantee of payment or coverage.

If you do not have insurance or if you have an insurance plan with which we do not participate, payment is due in full at the time of your visit.
